Implementing Clinical Nurse Leader to improve patient safety

The purpose of every health care center is to improve the health of their patients. It is the duty of all CNLs to strive and have unique credentials in their working places. This working place is what is referred to as clinical setting. Possessing positive, unique credentials is necessary for the safety of a patient being handled. To execute this well, all practices carried out must be based on evidence. A Clinical Nurse Leader is expected to bring out a high level of clinical competence and wisdom so as serve their clients with the best services possible. (Harris, J. L., et al. 2024)

It is then wise while handling any patient, to evaluate the patient outcomes, assess their cohort risk, and if there is any care plan deem unfit, it is their responsibility to request the decision-making authority to change it. They also form part of the interdisciplinary team, although their roles usually change depending on the different settings. (King, C. R.,et al. 2023).

Evidence-based practices (EBP) are very fundamental in the nursing profession. It is emphasized everywhere; in learning institutions and the research field. It is stressed so that to give a new lime light in terms of care, safety and efficiency. For this to be successful CNL should take on; practice adoption, education and circular realignment, model and theory development and more engagement in the field of research. . (Harris, J. L., et al. 2024)

Improving patient safety

 Improve hand hygiene compliance.

The number one course of slow healing rate globally is unhygienic conditions in hospitals. Statistics has indicated that 39% of health care infections are as a result of dirty hands. This has been so while all doctors and nurses know very well the importance and value of safety and prevention measures. It is the duty of CNL to implement their leadership quality and assign nurses to educate patients on when and how to wash their hands. Also, other hygienic conditions that need to be observed are Alcohol-based rubs and gloves. They should always be available to the nurses, doctors and patients. . (Harris, J. L., et al. 2024)

  • Use barrier safety measures to stop the spread of diseases.

It has been noted that if barrier precautions and hand hygiene are observed then there is a possibility of reducing nearly 2 million people suffering from health care infections annually in US only. When a patient has a serious nosocomial disease such as vancomycin-resistant Enterococcus, health workers should wear gowns and gloves when providing care. A care bundle including barrier safety measures has been revealed to reduce the spread of VRE from 0.21 to 0.09 in intensive care units. (King, C. R., et al. 2023).

  • Avoid dangerous drug abbreviations.

Every day in health care centers a mistake is done following confusing abbreviations such as q.d for daily. It should be avoided, or otherwise digital physician order entry systems also can help eliminate the vestiges of this problem. (King, C. R., et al. 2023).

  • Implement care bundles to avoid central line-related bloodstream illnesses.

Using barrier safety measures also is a tool in the procedure that dramatically can reduce the rate of central line-associated bloodstream infections. When putting central venous catheters or peripherally inserted central catheters, health professionals should put on caps, masks, sterile gowns and hand protection, and a whole body drape should be put on the patient. (King, C. R., et al. 2023).

Specific plans to extend a successful project solution

Creating a successful project is the most critical thing to achieve. Once a project is successful, that becomes the beginning of victory. However, successful projects need to be maintained and observed for them to flourish. It can be achieved by: creating a project success plan and project success management. The main aim of creating a project success plan is to make sure that you manage all the communications surrounding the project. (Lechler, T. G. 2025).

i)    Serve as the middle person and try to make every member friendly to each other.

ii)    Discuss and arrive at a conclusion on how to set criteria for achieving success.

iii)    Discuss and share responsibilities.

iv)    Every individual should be allowed to speak out their minds.

Methods and specific plans to revise an unsuccessful project solution

It is not a new thing if a project turns out to be unsuccessful. The most important thing is to create particular plans for revising the project that has fallen. This is how this can be done

i)    Refocus the Scope

Here what is done is revision of the previous project. The charter that was previously created, statement of the work and approved change requests. Figure out your accomplishments and commitments. Meet the sponsors and explain to them. (Lechler, T. G. 2025).

ii)    Draw up the schedule

Determine the timeframe that will take you to finish the project having done away with the previous deadlines. Calculate everything based on the remaining time and resources.

iii)    Determine the cost

Calculate the budget afresh from the available cash.

iv) Review lessons learned

From the past committed mistakes learn and educate yourself to avoid them.

v)    Develop alternatives

From the past mistakes, it is obvious to identify what messed the project. This time round, it is wise to create alternatives. If the budget also is not enough, figure out ways of catering for the lost capital. (Tian, X., et al. 2024).

vi)   Admit reality

Even if there is very list to expect from this project, it is good to admit reality and move on. (Tian, X., et al. 2024).

vii)    Start afresh

Once everything has been set on toes, get ready and start the project.

Methods and specific plans to terminate an unsuccessful project solution

Terminating a project is always not the best thing to do. Although there might be circumstances, rendering you helpless and the only remaining option is terminating the project. If this happens, then the first thing is to meet your supervisor or administrative representative(s). Discuss with them the potential of the project. This should always be the case even if the conditions that have led to the termination come from within or outside the nursing organization. (Bender, M. 2023).

While meeting with them, inform others who have worked on the project to be there. It is better for them to know that a project is being terminated since they have invested their time, energy, and others money. ( Bender, M. 2023).

The members who worked on the project should not be blamed on threatened with a disciplinary action. Every project is doomed to succeed or fail.( Bender, M. 2023).

The feedback of a project is known through different methods. (Lechler, T. G. 2025).

i)    First, is the reaction of the targeted group. For example; if the project aims at improving patients safety then what is monitored is the patient’s safety currently compared to the beginning of the project. (Tian, X., et al. 2024).

ii)    Secondly, you can observe the budget if it is by your plan. (Tian, X., et al. 2024).

Barriers in a clinical Setting

i)    The first stumbling block is ignorance. Many people are ignorant of their safety as patients. This barrier can be overcome by educating the patients on how to maintain their safety.

ii)    The second stumbling block is the lack of motivation by the team members; this can be overcome by being friendly and informing the team members on the importance of teamwork. (Lechler, T. G. 2025).

Recommendation

The CNLs should be self-motivated, execute their leadership qualities and do a lot a lot of research to save people’s life. The government should also cheap in and fund them so as to achieve a healthy population shortly.

Conclusion

Putting in place the Clinical Nurse Leaders will be a good start in attain a healthy and a safe environment for patients in hospitals. Provided they maintain unique credentials and thirst for work and research, there is an assured patient safety shortly.

Discussion questions

i)    How can patient’s safety be improved?

ii)    What is the importance of evidence-based practices?

iii)    List down the processes of revising an unsuccessful project.
